Reliability & Maintenance
Apollo Root Cause Analysis™ Methodology — Solve Complex Problems and Prevent Recurrence
A two-day, hands-on programme for anyone who investigates incidents or recurring failures. Day 1 builds the Apollo RCA™ principles — separating symptoms from causes, cause-and-effect logic and evidence. Day 2 is spent working real scenarios with RealityCharting® software, ending in solutions that stop the problem coming back.
Industry focus: Plant Maintenance, Reliability & Inspection
Who Should Attend
Designed for these roles
- Engineers in design, process and systems roles
- Incident investigators and EHS managers
- Maintenance, reliability and quality assurance personnel
- Operations, risk, compliance and process improvement professionals
Delivery formats
- Customised in-house training at your plant or office
- Public classroom delivery, subject to schedule
- Live online delivery where suitable
- Recommended group size: 8–20 participants
Problems Solved
What this course fixes
Investigations that stop at a symptom or 'human error' and never reach a root cause
The same incident or failure repeating after the report is closed
Blame-focused investigations that shut down honest reporting
Investigation teams with no shared method or format
Learning Outcomes
What participants can do after the programme
Outcomes are written as tasks, not topics — so managers can verify capability back at work.
- Explain the principles of the Apollo Root Cause Analysis™ methodology and effective problem solving
- Define problems precisely and separate symptoms from root causes
- Build evidence-based Cause & Effect Charts for complex problems
- Use RealityCharting® / RC Pro® software to support and document the analysis
- Facilitate group problem-solving sessions without placing blame
- Evaluate potential solutions and select those that prevent recurrence
- Communicate findings and recommendations clearly to stakeholders
- Integrate Apollo RCA into existing organisational processes

- Defining the problem: what, when, where and significance
- Cause-and-effect principles and causal relationships
- Evidence-based analysis and building the Cause & Effect Chart
- Introduction to RealityCharting® / RC Pro® software
Day 2 — Applying Apollo RCA to Real Scenarios
- Facilitating a group RCA session
- Case studies and participants' own incidents worked end to end
- Identifying and evaluating effective solutions
- Reporting results and communicating to stakeholders
- Embedding RCA in the organisation for continuous improvement
Trainer Profile
Delivered by Sathish Kumar
Senior Trainer — Reliability, Maintenance & Rotating Equipment
19 years industry experience
Sathish builds maintenance strategies that survive contact with a real budget. His workshops use participants' own failure history, so teams leave with a revised task list rather than a certificate alone.
Real project exposure
- RCM implementation across critical pumps and compressors at a combined-cycle power plant
- Root cause failure analysis on repeat bearing failures in a boiler feed pump train
- Spare parts and criticality review for a palm oil refinery group
Certifications
- CMRP — Certified Maintenance & Reliability Professional
- Vibration Analyst Category III (ISO 18436-2)
- RCM2 Practitioner
